Medical Home Port


The Medical Home Port (MHP) staff makes it their mission to provide compassionate and structured care to help prevent disease and illness. MHP provides general physical examinations and primary/well care for adults up to the age of 65 and children, including infants and newborns.  

Medical Home Port Services

Our goal is to provide innovative and person-centered care. The services provided at MHP include:
  • Complete medical evaluations of patient's symptoms
  • Electrocardiograms (ECGs)
  • Endometrial biopsy
  • Blood pressure checks (for enrolled patients)
  • Adult/Pediatric screening visits
  • Cryotherapy
  • Suturing and suture removal
  • Nebulizer Treatments
  • Education for diabetes, hypertension, asthma, cholesterol, and other diseases and conditions
  • School and sports physicals
  • Behavior and development problems
  • Referral consultation to specialty clinics and providers for services beyond the scope of care of the Family Medicine Clinic
  • Asthma and diabetes evaluations and consultations
  • Internal behavioral health consultation - triage and consultation for short-term behavioral, occupational, diagnostic and treatment-related issues
  • Routine physical exams and pap smears
  • Colposcopy referral
  • Punch, shave, and excisional biopsies
  • Acute evaluations, routine infant/child health checkups
  • Medical/Medication assistance via telephone during clinic hours

Walk-in Services

Medical Home Port offers two walk-in clinic services: Sick Call for All Acute Care Walk-in Clinic and Contraceptive clinic

  • Sick Call for All Acute Care Clinic

    Mondays and Fridays: 7:40 a.m. - Noon

  • Contraceptive Clinic

    Fridays 9 a.m. - 11 a.m.
